The Community Habilitation program is designed to support participants in developing the skills necessary for independent living within their community. Our program serves individuals who are Deaf, Hard of Hearing, or Hearing with Intellectual and Developmental Disabilities. Through participation, individuals are empowered to be who they are by enhancing various skills, such as independent living, socialization, communication, financial management, health literacy, and community integration.

Community Habilitation is a personalized service in which individuals collaborate with their team, including a Care Manager and a Community Habilitation Direct Support Professional (DSP), to create a tailored plan that promotes personal independence and growth. The plan is developed based on each individual’s preferences, goals, and needs.

Our program adheres to the guidelines set by the New York State Office of People with Developmental Disabilities (OPWDD) and is primarily funded through the HCBS Medicaid waiver.
